Case management

Case planning is a cornerstone of our program, and El’dad specializes in providing expert support to individuals with complex psychosocial needs.

This starts with the creation of a Circle of Care plan. These plans address individual-specific needs across physical, emotional, mental, and spiritual dimensions. These plans, modeled after the Indigenous Medicine Wheel, reflect a holistic support system in which participants can achieve their personal goals.

Participants are directly involved in the creation of their plans, which incorporate their own beliefs and practices. Through this person-centered approach, participants and staff are learning more about the disabilities and neurodiversities that participants live with. Together, we are setting goals and prioritizing training around FASD, Autism, ADHD, and intellectual disabilities. Our Case Management team is working to build staff and participant capacity around Executive Functioning, Memory (sensory, short-term, long-term, and working memory), Sensory Integration, and Auditory Processing. Circle of Care plans are the vehicle to build capacity in these areas.
